Clarification regarding numerator requirements for triglycerides including or excluding and denominator risk factor count

      Question 1: The following has been mentioned in the header of the measure:
      Numerator 1:
      Patients whose most recent fasting LDL-C test result is in good control, defined as <100 mg/dL
      Numerator 2:
      Patients whose most recent fasting LDL-C test result is in good control, defined as <130 mg/dL
      Numerator 3:
      Patients whose most recent fasting LDL-C test result is in good control, defined as <160 mg/dL

      However, the Numerator 1, Numerator 2 and Numerator 3 in the population criteria requires LDL-C test along with Triglycerides to be documented to satisfactory reporting of the numerator:

      Can you please confirm the criteria required for satisfactory reporting of the numerator?
      • Numerator 1: LDL lab test completed with result < 100 mg/dL during measurement period AND Triglycerides test completed with result <= 400 mg/dL during measurement period
      • Numerator 2: LDL lab test completed with result < 130 mg/dL during measurement period AND Triglycerides test completed with result <= 400 mg/dL during measurement period
      • Numerator 3: LDL lab test completed with result < 160 mg/dL within 4 years prior to the measurement period AND Triglycerides test completed with result <= 400 mg/dL within 4 years prior to the measurement period
      • Please specify If it is other than specified above 3 criterias

      One of the conditions to meet the denominator 2 of the measure requires count three or more of the risk factors AND count 2 of the same risk factors. Can you please confirm if it should be an OR operator instead of AND?
