      The measure names are Stage 1 Transition of Care summary Menus 8 and Stage 2 Summary of Care Core 15. See measure details below:
      Question: The denominator is looking for Number of transitions of care and referrals during the EHR reporting period for which the EP was the transferring or referring provider. And the numerator is looking for number of transitions of care and referrals in the denominator where a summary of care record was provided. Thus, the question is in reference to how the providers are credited for this measure. Does the provider for the transitions of care and referrals in Denominator have to be the same provider that provides the summary care record for the numerator?
