      We are trying to determine how providers are credited measure performance for each measure. We want to understand if the CQMs require a provider to perform all clinical actions in order to meet performance for each measure or will all providers receive credit as long as long ‘a’ provider performs the clinical actions.

      For example CMS74v3: All providers (A,B,C) are from different practices that are part of one enterprise. Thus, Practice 1, Practice 2, and Practice 3 are within one enterprise. From below sample will All Providers A, B and C receives credit for the numerator action performed by Provider C since they are all part of the same enterprise? As data will be shared across the three practices as all the practices are part of one enterprise; even though Provider A and B never documented the Fluoride Varnish Application Procedure in patient chart. Or will only Provider C receive numerator credit as this provider performed and document ted the clinical actions for numerator.

      • Provider A: sees Patient 1 during the measurement period on 1/1/14 with a valid encounter code and no documentation of Fluoride Varnish Application Procedure. This provider is from Practice 1.
      • Provider B: sees Patient 1 during the measurement period on 8/1/14 with a valid encounter code and no documentation of Fluoride Varnish Application Procedure. This provider is from Practice 3.
      • Provider C: sees Patient 1 on 6/1/14 with a valid encounter code and performs Fluoride Varnish Application procedure. This provider is from Practice 2.
