    • Thank you for your question. If a patient is on a Direct Thrombin Inhibitor he or she will be included in the numerator of NQF0371. For the 2014 EH annual update the logic was revised to clarify this.
    • CMS108v1/NQF371

In the NQF0371 numerator, there are multiple references to medications used for VTE prophylaxis, specifically unfractionated heparin, low molecular weight heparin, factor Xa, and warfarin. However, in one of the subgroups within the numerator there is a reference to direct thrombin inhibitor. This group does not include warfarin. The associated refusal and medical reason criteria refer to warfarin, not direct thrombin inhibitor. Can you confirm that the inclusion of direct thrombin inhibitor was an error?

      Section of numerator:

■OR: ■OR: "Medication, Administered: Unfractionated Heparin (route: 'Subcutaneous route')"
■OR: "Medication, Administered: Low Molecular Weight Heparin"
■OR: "Medication, Administered not done: Patient Refusal" for "Unfractionated Heparin RxNorm Value Set"
■OR: "Medication, Administered not done: Patient Refusal" for "Low Molecular Weight Heparin RxNorm Value Set"
■OR: "Device, Applied not done: Patient Refusal" for "Intermittent pneumatic compression devices (IPC) SNOMED-CT Value Set"
■OR: "Device, Applied: Graduated compression stockings (GCS)"
■OR: "Medication, Administered: Unfractionated Heparin (route: 'Intravenous route')"
      ■OR: "Laboratory Test, Result: INR (result > 3 )"
■OR: "Risk Category Assessment: VTE Risk Assessment (result: 'Low Risk')"
■OR: "Medication, Administered: Direct Thrombin Inhibitor"
