electronic Long-Term Services and Supports (eLTSS) Overview

The electronic Long-Term Services & Supports (eLTSS) Initiative launched on November 6th, 2014. This initiative is an ONC-CMS partnership focusing on the identification and harmonization of electronic standards that can enable the creation, exchange and re-use of interoperable service plans for use by health care and community-based long-term services and supports providers, payers and the individuals they serve. These plans can help to improve the coordination of health and social services that support an individual’s mental and physical health.

This initiative is driven by the requirements of the CMS Testing Experience and Functional Tools (TEFT) in community-based long-term services and supports (CB-LTSS) Planning and Demonstration Grant Program created in the Affordable Care Act (ACA).

HL7 September 2020 Virtual Connectathon 

Proposed eLTSS FHIR IG STU Update

An eLTSS Community Update was conducted on Thursday July 16, 2020 

  • This update included a report on eLTSS testing at the HL7 May 2020 Virtual Connectathon and an outline of plans for eLTSS testing at the HL7 September 2020 Virtual Connectathon
  • Also on this update representatives from the State of Missouri shared their takeaways from the May Connectathon, and Patient Centric Solutions demonstrate their implementation of eLTSS within their PatientShare system

eLTSS FHIR Implementation Guide The eLTSS FHIR Implementation Guide (IG) Release 1 - US Realm is based on FHIR R4. It was developed to support the exchange of data generated during the planning and provision of long-term services and supports and is currently scoped to data commonly found on LTSS service plans.  
eLTSS Reference Implementation

The eLTSS Reference Implementation supports the establishment of an eLTSS FHIR R4 server which utilizes the eLTSS FHIR Implementation Guide and provides five eLTSS sample data bundles for testing

eLTSS Informative DocumentThe HL7 Cross-Paradigm Information Sharing for Electronic Long-Term Services & Supports (eLTSS), Release 1 - US Realm Informative Document was balloted through the September 2018 HL7 ballot cycle. This document describes how Home and Community-Based Services (HCBS) data requirements, documented in the 56 data elements included in the eLTSS Dataset, can be represented for exchange and sharing using nationally recognized content and vocabulary standards.
eLTSS Dataset C-CDA MappingThe Georgia Team mapped the Final eLTSS Dataset to HL7 C-CDA Release 2.1. This mapping is included as an appendix in the eLTSS Informative Document balloted through the September 2018 HL7 ballot cycle and will be updated as needed to address ballot comments.

Final eLTSS Dataset & Summary

The final eLTSS Dataset is the culminating deliverable of the eLTSS Initiative which is comprised of core and non-core data elements. This dataset was developed and vetted through two rounds of piloting, harmonization and disposition activities.

The Summary includes a high-level overview of the creation of the eLTSS Dataset, methodology, and vision for dataset integration.

The Executive Summary is a high-level overview of the eLTSS initiative, covering aspects from inception, to execution of piloting and harmonization activities.

The eLTSS Initiative Summary is a detailed review of the eLTSS initiative, describing all aspects from inception, to execution of piloting and harmonization activities.

eLTSS Round 1 Published DatasetThis is the final Harmonized Dataset that was consented by the eLTSS Initiative Community during Round 1 of piloting. Definitions for each data element is included.
eLTSS Project Charter

The document describes the overall eLTSS project charter, including the challenge statement, scope, deliverables and timelines.

eLTSS Use Case

A use case is a methodology used in system analysis to identify, clarify, and organize system requirements. The eLTSS Use Case is developed by community input framed by the Project Charter content.

eLTSS GlossaryA community-driven glossary that was actively developed during Round 1 of eLTSS. Final updates were made August 2015.
