What is required and where should data populate

      Hello,

      I am working on refining our acute care inpatient nursing documentation - specifically the admission assessment.  I am trying to understand the USCDI measure requirements and what is the expectation of where the data populates ... for example the CCD.  I have example below.  I would love to speak with someone directly about this if possible.
