For the 2014 reporting year, from the CMS eCQM library website (http://cms.gov/Regulations-and-Guidance/Legislation/EHRIncentivePrograms/eCQM_Library.html, the QRDA Information section), please download the 2014 CMS QRDA implementation guides for Eligible Professionals Clinical Quality Measures and the 2014 CMS QRDA Implementation Guide for Eligible Hospital Clinical Quality Measures (Effective July 1, 2014) and see specific CMS requirements for TIN and CCN.

In the header section for providers (performer > assignedEntity), there is a place for each provider's NPI to be specified. There is also a representedOrganization tag that has places where the org's TIN and CCN (CMS Certification Number) can be specified. These aren't required by the QRDA Cat I schema, but I'm wondering if CMS is going to want to see them in submissions nonetheless?
