HIE objective measure denominator should exclude inpatient discharges to new level of care at same facility/unit/provider

      At serious risk for not achieving 50 Promoting Interoperability points due to objective measure denominator interpretation. HIE ojective measure "Sending Health Information" denominator reads "at least one transition of care or referral". As a CAH, we must discharge Inpatient accounts to readmit into Swing Bed accounts, thus allowing for separate billing. I argue this is a change in inpatient level of care, not a transition and should be excluded in our denominator calculation; however, our PI measure vendor needs documentation from measure stewards to allow us to exclude this subset of discharged inpatients (discharge disposition = transfer to SWB). Of note, discharge from swing bed would still be included in our denominator
