Issue is with exception reporting for Stroke 2 and Stroke 3 All anticoagulants are in the antithrombotic RX Norm Valueset. This creates an issue for clients reporting Stroke 2 and Stroke 3. If reporting both then the patient. Providers routinely document a reason for not giving an anticoagulant, but when you look at the QRDA file it often shows they were prescribed an antithrombotic AND they have a reason for not prescribing an antithrombotic. QRDA took what was a reason for not giving an anticoagulant and translated it to be antithrombotic reason.
Suggested solution would be to have a unique RXNorm code per value set.
