"No Patient Identifier" error while uploading QRDA I file.

      Hello Team,
      We need urgent help for QRDA I 2018 submission.
      When uploadin QRDA I file on healthmonix, it fails giving error "No Patient Identifier".
      Please refer the screenshot . We include the identifier in this way.

      Is this correct or we are missing something?
