Customer have reported complaint as QRDA III QRDA Category III_NoPHI.xmlupload on CMS website is failed.

Same QRDA III report is passing on Cypress after upload.

After investigation we found below error in error logs of CMS, provided by respective customers :

First instance 'I1' from customer site 'A' :

Second instance 'I2' from customer site 'B' :

For site 'A' :

   Cumulative performance of NQF 0022 is

   10/406 (for population 1) +4/406 (for population 2) = 14/812

FOR site 'B' :

    Cumulative performance of NQF 0022 is

    0/293 (for population 1) +0/293 (for population 2) = 0/586

I have attached QRDA III report  QRDA Category III_NoPHI.xmlfrom one of the customer site for your reference.

Both instances of issue have similar error message. 

So that, by looking towards the error message, we feel that NQF 0022 performance is not valid as per CMS therefore report is failing.

Our questions are :

Q1. Is there any specific expectation from CMS about performance of NQF 0022 to pass the QRDA III report? If yes then what is it?

Q2. If this issue is not related to NQF 0022 performance then what will be the issue/area of issue?

Q3. Is there any problem from our side while calculating cumulative performance or while showing performance numbers in QRDA III report?

Q4. Is there need to show only cumulative performance numbers in QRDA III report for NQF 0022 instead of separate performance numbers ?
