For 2017 performance period, all eCQM version specific identifiers and population identifiers are listed in the 2017 CMS QRDA III Implementation Guide for Eligible Clinicians and Eligible Professionals Reporting v1.0 (https://ecqi.healthit.gov/system/files/eCQM_QRDA_EC-508_0.pdf), starting on page 87 (Chapter 6).
