Whose name should be under the organization name? Should it be the organization who runs the reports (the EMR hosting organization) or the one that is reporting the information?

I've been trying to find the importance and significance of the "Document maintained by" field when submitting to QRDA. If we change the name of the organization, that field automatically changes. What would happen if it was a partner organization who was reporting under their name, but they were not the ones, technically, maintaining the document? Would that make a difference?
