Received errors on all files when uploaded.

      Please consult the 2017 CMS QRDA-III IG https://ecqi.healthit.gov/system/files/eCQM_QRDA_EC-508_0.pdf. To submit for 2017 reporting you must conform to the 2017 IG.

      We noticed right away that there were some things not adhering to the 2017 IG:
      It looks as if the template IDs in the sample file are not valid for 2017 and are from an older version of the CMS QRDA-III IG.
      CMS Program name PQRS_MU_INDIVIDUAL is no longer valid in 2017 reporting.

      Received 'Clinical Document program name is not recognized' and 'The Quality Measure Section must have only one Reporting Parameter' errors on all files uploaded to portal. Any guidance on how to modify reports would be appreciated.
