Clarification of information on report EHR Hospital Reporting - eCQM Submission and Performance Feedback Report

      Thank you for submitting your JIRA ticket. We advise you to contact the CMS HQR submission tool help desk at 717-718-1230 extension 105 and submit a ticket for the QualityNet tool to document and track.       In short, the errors you received were not generated by the PSVA tool. Instead, the errors you received on the last report were generated by the HQR HITECH/EHR Measures Engine. PSVA only partially validates the QRDA file. It does not validate the measures data and some of the fields that require database for lookup values. Also, data accepted successfully by the CMS test server would still fail in production if some of the fields like dummy CCN are not corrected prior to production submission. Passing PSVA validation does not indicate that the submitter has formulated a QRDA that will give them an IPP of 1 or a numerator of 1 for a measure. Although the user may believe the patient has met the numerator or met the IPP, the QRDA has to contain the correct information in it, as well as, the right location in the QRDA file for the Measures Calculations to determine that. Therefore, We suggest that you work with Beth Driskel to determine if she can help you formulate a QRDA that can be used successfully in measure calculations.

      This is regarding test QRDA Cat I files that we submitted to CMS via the PSVA tool in preparation for our CY 2016 eCQM February 2017 Live submission of either 2016 Q3 or 2016 Q4 data.

      According to our Meditech C/S 5.67 report results, several accounts in our test data meet all numerator requirements for both the Hearing Screening (EDHI-1a) and Exclusive Breast Milk Feeding (PC-05). A QRDA Cat 1 test file was produced and uploaded via the PSVA tool. It passed the validation stage and was then submitted to CMS (Test).

      In QualityNet, I ran the submission summary reports showing that all files were accepted (no rejections).

      I then ran the following report in QualityNet:
      Inpatient Hospital Quality Reporting Program > My Reports
      Report Program - IQR
      Report Category - EHR Hospital Reporting
      Report Name - EHR Hospital Reporting - eCQM Submission and Performance Feedback Report

      For all EDHI-1a records, the report states, NUMERATOR NOT MET
      For all PC-05 records, the report states, INITIAL PATIENT POPULATION NOT MET

      There were several patients that met both the initial patient population and numerator according to our Meditech reports. Why is this CMS report showing otherwise?
