Clarification of the terms Author, Custodian, Legal Assigned Entity, and ONC Certification Number

      For the 2016 Reporting Period, please refer to the 2016 CMS QRDA IG and 2016 CMS QRDA IG Appendix for more detailed reporting requirements in general, both could be downloaded from the CMS eCQM Library page under the QRDA RESOURCES section.

      1) Who is the Author?
      Author represents the creator of the QRDA-I file. The author maybe a device or a person. If you are using eCQM SQL scripts provided by Meditech to produce QRDA-I files, author could be reported using assignedAuthoringDevice with Meditech product information.

      2) Who is the Custodian?
      For hospital reporting, custodian must be the CCN number. See the 2016 CMS QRDA IG. Section 5.1.3 Custodian for details.

      3) Who is the Legal Assigned Entity?
      legalAuthenticator could be the name of quality nurse or CIO who reviewed the report from your hospital, as an example.

      4) Is the ONC Certification Number our site's 6-digit CMS Certification Number?
      ONC Certification Number is different from CCN number, ONC Certification Number is not required by CMS reporting, it does not need to be provided.

      We will be attempting to produce QRDA I files for CY 2016 eCQM Reporting.
      Meditech Client/Server 5.67 is our EHR vendor who has provided the eCQM SQL scripts for all 2015 EH CQM measures as well as a tool to produce the QRDA I xml files.
      We are planning to submit the eCQM QRDA files ourselves without the benefit of an outside vendor.
      Taking the above information into consideration,
      1) Who is the Author?
      2) Who is the Custodian?
      3) Who is the Legal Assigned Entity?
      4) Is the ONC Certification Number our site's 6-digit CMS Certification Number?
      There are several SQL parameters requested in Meditech's configuration parameter table pertaining to Author, Custodian, and Legal Assigned Entiry in order to produce a valid QRDA I file. We are having difficulty interpreting the eCQM CMS specifications as to what should go into these parameter fields. I have also searched for similar issues on this site and it is still not clear as to what we should supply for these parameter values for our particular situation.
