There are templateIds required by the CMS IG. The presence of those templateIds are disallowed by the QRDA IG

      Both @root and @extension together makes a unique template id. The 2016 CMS QRDA IG uses different @extension to indicate this is a CMS version of the template; it does not contradict the HL7 QRDA template, it conforms to the HL7 QRDA-I template that it is derived from instead. The published 2016 CMS QRDA IG schematrons (downloadable from eCQM Library website) validates the template ids correctly, the Cypress validator may not have yet been using these schematron rules.

      The CMS IG requires multiple versions of certain templateIDs to be present in QRDA I documents for 2016 (they require an older version and a "2015-07-01" version with the same templateId. If all the templateIds required by CMS are included, the SITE tool throws the following QRDA errors:

      SHALL contain exactly one [1..1] templateId (CONF:23-18098) such that it SHALL contain exactly one [1..1] @root="2.16.840.1.113883.10.20.17.3.8" (CONF:23-18099).
      SHALL contain exactly one [1..1] templateId (CONF:23-14611) such that it SHALL contain exactly one [1..1] @root="2.16.840.1.113883.10.20.17.2.1" (CONF:23-14612).
      SHALL contain exactly one [1..1] templateId (CONF:1140-12796) such that it SHALL contain exactly one [1..1] @root="2.16.840.1.113883.10.20.24.2.1" (CONF:1140-12797).

      If the "2015-07-01" version of the templateIds are removed, the SITE tool doesn't throw any QRDA errors, but the following CMS errors are thrown:
      This structuredBody SHALL contain at least one [1..*] component (CONF:1182-28474) such that it SHALL contain exactly one [1..1] Reporting Parameters Section - CMS EP & HQR (identifier: urn:hl7ii:2.16.840.1.113883.10.20.17.2.1:2015-07-01) (CONF:1182-28475).
      This structuredBody SHALL contain at least one [1..*] component (CONF:1182-28476) such that it SHALL contain exactly one [1..1] Patient Data Section QDM (V2) - CMS EP & HQR (identifier: urn:hl7ii:2.16.840.1.113883.10.20.24.2.1:2015-07-01) (CONF:1182-28477).
      This structuredBody SHALL contain at least one [1..*] component (CONF:1182-28476) such that it SHALL contain exactly one [1..1] Patient Data Section QDM (V2) - CMS EP & HQR (identifier: urn:hl7ii:2.16.840.1.113883.10.20.24.2.1:2015-07-01) (CONF:1182-28477).
