For the 2015 reporting year, which uses the 2015 CMS QRDA IG, you could find the list of eCQM version specific identifiers and population identifiers for each EP eCQMs (and identifiers for stratifications if applicable to that eCQM) in the Addendum to the 2015 CMS QRDA IG.
