NEXTGEN Questions submitted 11/22/13 - CAT III and CMS measures

Question 1#: For each CMS measure Supplemental Data Elements for demographic information is indicated below, should we only include patients with the all Supplemental Data Elements documented in the measure counts? Or are these criterions optional?

Question 2#: CAT III files: A patient may have multiple races documented. How should this be displayed/counted?
Example: Patient is African American and Asian. Should this patient be counted once for each race? Or should they just be displayed in one , if so which one should they appear in?

Question 3#: CAT III files: Can you please provide us the most updated example of the CAT III? Previously our files did not identify payer, race, and ethnicity as it was not required. However, we attempted to trial our files via Cypress and we are receiving errors for the files due to the missing demographic information. But our measure counts are displaying correctly. If you are the not best resource to answer these questions, can you please better direct us a resource that can assist with this?
