QRDA reporting based on IPP or denominator?

    • Per the 2016 CMS QRDA IG, "A QRDA-I document should be submitted for each patient who meets the Initial Patient Population criteria of an eCQM." This was also stated in the 2015 CMS QRDA IG.

      The Denominator certainly narrows the population to the benefit of the performance rate, but what about QRDA Category I reporting? Per the QRDA specification, generation of QRDA Category I documents is determined by the IPP, not the Denominator. The EH Supplemental Implementation Guide v2.1 overrides this default, such that QRDA Category I documents are submitted only for patients meeting the Denominator, but the EP Supplemental Implementation Guide (v3.0 appears to be the latest) provides no such override. Thus this measure appears to require the submission of a QRDA Category I document for every patient 18 or older, even if, as the reporter noted, the patient was not even seen by the provider during the measurement period. Any guidance on that or is that indeed the expectation?

      Here is the reference from the latest implementation guide (V 4.0 released on 4/18/2014) section 4.1: “A QRDA-I document should be submitted for each patient who meets the Initial Patient Population criteria of an eCQM”. This results in having to generate thousands of QRDA 1 files for this measure versus a much smaller number if more filters are applied to the IPP.
