      As the submitter correctly points out, guidance in the QRDA-I is that

      "for disjunctive criteria, e.g., where a criterion can be satisfied by either of two data elements, include smoking gun data for both data elements".

      In this case, one should in fact use smoking gun data for both criteria.

      if there is more than 1 way of meeting the numerator of any measure and the patient meets both the ways, do we show both the numerator criteria on the QRDA I or any one of them? Also if there are 2 ways to exclude a patient and either of them is sufficient, do we need to show both the exclusions on QRDA I if patient meets both the exclusion criteria?
