Please provide assistance on following question based on QRDA 3.

Q: What if patient gender is not entered? Will it be a part of total population count but not any of the aggregated count i.e. Male, Female or Undifferentiated.

Q: What if patient race is not entered? Will it be part of "Other Race" (Code 2131-1) or simply not considered in aggregated count template but still part of total population count.

Q: If patient has multiple race, then how to classify such patient? From previous posting I found that we have to put such patient in "Other Race" bucket.

Q: What if patient Ethnicity is not entered? Will it be a part of total population count but not any of the aggregated count i.e. "Hispanic or Latino" or "Not Hispanic or Latino"?

Q: What if patient Ethnicity is not one of mention in IG i.e. "Hispanic or Latino" or "Not Hispanic or Latino". Can you provide mapping guideline or vendor can make decision on its own?

Q: What if patient payer is not entered? Will it be part of population count but not any of aggregate count i.e. Medicare, Medicaid, Private Health Insurance or Other.

Q: What if patient has insurance but its not marked as primary, can you put such patient in "Other" bucket?

Q: What if patient gender is not entered? Will it be a part of total population count but not any of the aggregated count i.e. Male, Female or Undifferentiated.

If the patient has no gender information, and if the patient meets the population criteria (e.g., IP, NUM, etc.), it will be counted in the population (e.g., IP, NUM, etc.). But the patient does not get counted in the aggregated counts (for each gender code) for administrative sex supplemental data element.

Q: What if patient race is not entered? Will it be part of "Other Race" (Code 2131-1) or simply not considered in aggregated count template but still part of total population count.

Same as for gender. If the patient has no race information, and if the patient meets the population criteria, it will be counted for that population. But the patient does not get counted in the aggregated counts (for each race code) for the race supplemental data elements.

Q: If patient has multiple race, then how to classify such patient? From previous posting I found that we have to put such patient in "Other Race" bucket.

Yes, for multiple races, the patient will be counted toward “Other Race” bucket.

Q: What if patient Ethnicity is not entered? Will it be a part of total population count but not any of the aggregated count i.e. "Hispanic or Latino" or "Not Hispanic or Latino"?

Yes, ethnicity follows the same approach as gender and race described above.

Q: What if patient Ethnicity is not one of mention in IG i.e. "Hispanic or Latino" or "Not Hispanic or Latino". Can you provide mapping guideline or vendor can make decision on its own?

The codes must be drawn from the ethnicity value set 2.16.840.1.114222.4.11.837. This aligns with the value set that is specified in the eCQMs. No official mapping guidance is currently available.

Q: What if patient payer is not entered? Will it be part of population count but not any of aggregate count i.e. Medicare, Medicaid, Private Health Insurance or Other.

Without knowledge of your specific EHRs and data, we could only provide suggestion. If patient has no payer information entered, suggest analyzing your data to see if this indicates self-pay or some other scenarios. No payment listed will be using the payer grouping code “D”, it will be counted toward as aggregated counts for “D”.

Q: What if patient has insurance but its not marked as primary, can you put such patient in "Other" bucket?

Without knowledge of your specific EHRs and data, it will be difficult to provide a yes/no answer. If the patient has only one insurance documented, but without indicating as primary, you might need to evaluate whether this should be considered primary based on your knowledge of your system and data.
