We are seeking clarification on the correct process for submitting eCQMs when a hospital changes EHR vendors during a reporting quarter.
In our case, one of our hospitals transitioned from a legacy EHR to a new EHR in Q2 2025. We understand that for quarters outside the transition period, data can be submitted separately by each EHR system. However, for the transition quarter (Q2), we have seen conflicting guidance:
- Some posts indicate that the legacy EHR should transmit data to the new EHR, and the new EHR should submit a single combined QRDA file for the quarter.
- Other older posts suggest that both the legacy and new EHR can submit data for the same quarter, provided patient identifiers are unique.
Could you please confirm the current CMS-approved approach for this scenario? Specifically:
- Should the transition quarter be submitted as one combined file from the new EHR?
- Or is it acceptable for both EHRs to submit separate files for the same quarter?
