The ‘Notes’ column in ‘CSV_Mapping_Specification_Request_and_Response_Final_01032014’ spreadsheet mentions that Sub Measure Yes No Answer is required if the 'Method of Measure' column in the Attestation Data Reference Sheet is 'Yes/No' for the measure AND the measure is not excluded (refer the 'Notes' in the Attestation Data Reference Sheet)
Now 'method of measure' column has yes/no values only in core and menu reference sheet. Does that mean we shouldn’t include CQMs here?

a) Not Required for Core and Menu Measures
b) For CQMs - Required if the 'Exception required?' column in the Attestation Data Reference Sheet is 'Yes' for the measure
c) Required if the 'Exclusion Type' column in the Attestation Data Reference Sheet is 'Exception' for the exclusion
Now 'exception required' column is not seen in the CQM reference sheet but in the menu and core measures reference sheet. Also the valid values for 'Exclusion Type' are - 'Numeric' and 'Exception_Numeric' but point c looks for value 'Exception'

The ‘Notes’ column in ‘CSV_Mapping_Specification_Request_and_Response_Final_01032014’ spreadsheet mentions that Exception1 Numeric Answer is required if the 'Exclusion Type' column in the Attestation Data Reference Sheet is 'Exception' for the exclusion. Now the valid values for 'Exclusion Type' are - 'Numeric' and 'Exception_Numeric' but this looks for value 'Exception'
