1. If my practice submits 6 measures, including measure 128, will the total measure achievement points be reduced by 10 points?
2. If I submit more than 6 measures, including measure 128, will the total measure achievement points be reduced by 10 points?
3. If I submit more than 6 measures, including measure 128, but some measures do not have a historical benchmark, will the total measure achievement points be reduced by 10 points?
4. Will CMS create performance benchmarks on measures that don't have historical benchmarks after the measure is submitted by at least 20 groups?
