Current eMeasures specification standards are not able to instruct the EHR to evaluate the set (array) of medication entries for a patient and determine the duration that a patient is on that therapy.

      Current eMeasures specification standards are not able to instruct the EHR to evaluate the set (array) of medication entries for a patient and determine the duration that a patient is on that therapy. At this time, the logic appears to only be able to evaluate one coded element in a value set at a time, and the value sets are built using RxNorm “Semantic Clinical Drug” terms. These are too granular to allow the current QDM to represent assessment at the active ingredient or therapeutic category level.

      Currently, our only workaround is to provide detailed guidance as part of the measure specification for MUCD-19 (ADE Prevention and Monitoring: Warfarin Time in Therapeutic Range (TTR)).
