Remove "Measurement Start Date" and "Measurement End Date" in favor of "Measurement Period"

      Some MU2 measures currently use "Measurement Start Date" and "Measurement End Date", although these concepts were never documented in the QDM specification. These concepts are unnecessary since they can be modeled just as simply using timing operators with "Measurement Period".

      To encourage more consistency, we recommend removing "Measurement Start Date" and "Measurement End Date". The following table shows existing usage and recommended usage for MU2 measures:
