A "patient reported" or "proxy reported" source did not have an output location in the QRDA Cat 1 representation of the patient. The MAT could (and I believe still can) add a source attribute to a data criteria in the measure. The issue is that when you generate a QRDA Cat 1 for a patient that aligns with that measure, there is no place to put the source attribute. I don't know if this has been fixed in Cat 1. Essentially if you represent a concept in the measure logic you need to be able to store the associated data on the patient in the Cat 1 specification. If there's nowhere to store that data, then you're never going to see that data on a patient.
