Ability to express a measure as intended
Modeling ACO 38, Admission for Patients with Multiple Chronic Conditions raised some tricky issues:
We would like to identify patients who have two or more chronic conditions. A chronic condition is identified via:
· An active diagnosis in the year before the Measurement Period
· At least 2 outpatient visits OR one inpatient visit related to this diagnosis
We are hitting a wall at being unable to use the Count operator in conjunction with AND/OR – we’ve tried substituting Satisfies Any, Union, etc but are in need of some assistance.
