Remove Cumulative Medication Duration as an attribute in QDM

      QDM has a timing attribute of "Cumulative Medication Duration" for 2 datatypes - Medication, administration and Medication, dispensed

      CQL is a better way to address cumulative medication duration than as an attribute of QDM. I propose we remove the QDM attribute and create a new set of CQL conventions to address the need.

      Dispensing:
      Cumulative medication duration is calculated from the number of doses dispensed (Medication, dispensed) divided by the number of doses per day – and then add all dispensing events to come up with the total number of days covered by multiple dispensing events over a defined time period. QDM should provide the ability describe a dispensing event with attributes of doses and frequency.

      Administering:
      Cumulative medication duration can also be calculated from the number of administration events (Medication, administered) from the beginning of the first to the end of the last over the defined period of time.
