An indication of whether a measure is episode of care based or patient based is recommended for inclusion in the definition of a measure. This information would need to be captured in the Measuring Authoring Tool (MAT) as well as the XML representations (e.g. HQMF). Patient based measures are those that evaluate the care of a patient, and assign the patient to membership in one or more populations. All of the information in the patient record must be considered when computing a patient based measure. The criteria for inclusion of a patient in a measure population may require that conditions be satisfied during multiple episodes of care; for example, a diagnosis occurring in one episode of care and treatment happening in a subsequent episode of care. In contrast to patient based measures, episode of care measures evaluate the care during a patient-provider encounter, sometimes called an episode of care, and assign the episode of care to one or more populations. In an episode of care measure, the full set of episodes of care are identified in the Initial Patient Population, and are always designated by a specific occurrence. The December 2013 version of the QDM does not provide any mechanism to identify measures as either patient based or episode of care based. Additionally, there is currently no way to identify which data element specified within the measure specifies the episode of care. While these constructs may not need to be specifically defined within the logic of the QDM, since the definition of episode of care measures and the associated episodes was not included as part of the 2014 Stage 2 measures the QDM should at a minimum track its relationship with episode of care based measures and the associated measure definition standards.
