PC-02 Cesarean Birth - feedback regarding the time limits of the documentation

      This is feedback regarding the time limits of the documentation of the gravida, preterm, term, and parity documentation on the maternal patient.

      Currently, the 334 Version 4.0.000 specification states that the gravida, preterm, term, and parity documentation on the maternal patient be documented before PCMaternal."LastTimeof Delivery"(Encounter" (please attachment for screen shot). __ 

      We request that the time bound nature of these four responses be removed. 
      The rationale for this request is that sometimes maternal patients present to the hospital in advanced stages of delivery. In this instance, care is given to the maternal patient and the fetus(s) first above documentation. When this occurs, documentation regarding prenatal items above are not documented in the medical record ahead of the maternal delivery. Patient first, then documentation is real life. 

      We do recognize that as soon as the patient delivers, technically, some of the maternal patient’s numbers change. While this is something that complicates the measures regarding time, we do not believe it should still be a factor in eliminating documentation that may have taken place after the delivery. Many maternal units would utilize their maternal data in this way – using the pre-natal values- and do not update the values to coincide with the change after delivery. If anything, the gravida, preterm, term, parity number definitions could be altered to include that the values are to be the pre-natal values to add clarity and remove the time-bound nature. Gravida would be the only number that truly wouldn’t change after the maternal delivery and really could have this date/time limitation removed altogether.
