Compliance with CQMs is grossly underreported when annual wellness visits and/or follow up visits are excluded from qualifying visits, resulting in the need to manually track CQMs. This involves a significant amount of personnel and financial resources being diverted to a process that should be automated by the EHR.

    • Annual wellness assessment: preventive care

Greenway EHR has updated Practice Analytics, their reporting software, to exclude any visit that is billed with a -25 modifier.  In multiple conversations with the vendor, they have repeatedly told us it was done to be in compliance with ONC regulations regarding use of modifiers.  We initially contacted ONC for clarification, and received updated guidance which appears to clearly state that the visit itself can be used for reporting CQMs, and the modifier should be essentially ignored.  We forwarded this to Greenway, who again seems to have misinterpreted the updated guidance and continues to assert that these visits do not count for reporting purposes.

We request that ONC address this issue directly with Greenway EHR, as the majority of our annual wellness visits and follow up visits are being excluded from reporting due to this change.  As the primary intent of the wellness visit is to address the multiple qualityn measures, it is at best counter intuitive to exclude these visits, and could prove to have deleterious effects on the solvency of the practice.
