I have an idea to improve the accuracy of transfer of medication orders from the hospital: have the hospital pharmacy and post acute or retail pharmacy vendor electronically linked, or at least responsible for sending and receiving the medication orders. This cuts out some steps that can cause transcription or omission errors, since the med profile originates from the hospital pharmacy and med orders are ultimately received by pharmacy. Also, pharmacy staff are very familiar with ensuring the detail of accurate doses and frequencies.
Your article mentions electronically connecting hospital and post acute EMRS, but I've never seen my idea, to connect the pharmacies.
