      This measure is based on more subjective staging than objective. There are varying levels of expertise in the identification of and documentation of pressure injury which can lead to inaccurate data. This measure is also redundant to the current PSI 03.
      If this measure is considered, we request that Palliative Care patients be excluded from the population. It is not uncommon for patients in this population to refuse preventive measures.
