Exported QRDA-I document is missing facility location SNOMED-CT code and associated information in Encounter Performed template. The code is shown in the HTML version of the patient data but missing from the QRDA-I document.

An example of this issue can be found on the test patient for measure CMS9v5/NQF0480 with a Last Name of "PC5" and First Name of "184" using the Bonnie login "ron.peterson@ge.com".
