For example, we created a record that met the initial patient population with a Risk Screen result equal to " Malnutrition Screening Not At Risk". Bonnie produced an actual outcome of zero for ob1, zero for ob2, one for ob3, and one for ob4. We did not document any additional data elements, for example malnutrition dx or care plan. This also resulted in an actual of value of 2 in ob5 and a percentage of 200 in ob6. 

The expected outcomes were one for ob1, zero for ob2, zero for ob3, zero for ob4, 1 for ob5, and 100% for ob6.
