On 10/30/2015 we received an email informing us that version 1.4.0 of the Bonnie Tool was available for use and that this version of the tool supported QDM 4.2. I think that QDM 4.2 implies support for Release 3 of the HL7 QRDA 1 IG and, regardless, we have been independently assured that this version did support the Release 3 of the HL7 QRDA 1 IG.

When we ran the tool (which identified itself as v1.4, which we assumed was OK) and attempted to create QRDA 1 files consistent with the HL7 R3 IG we encountered significant problems.

We loaded measure CMS 157v4 and were able to define parameters for the measure and proceeded to create an output QRDA 1 file. Unfortunately, there were many problems with the QRDA 1 file it created.

For the most part, the header of the QRDA 1 file was Release 3 compliant except that an ‘extension’ attribute was missing for one of the initial template IDs in the file. The created header looked like… (first attachment)

This is missing the ‘extension=”2014-06-09”’ for template ID “2.16.840.1.113883.10.20.22.1.1”. The header should look like… (second attachment)

However, the bigger problem was with the created Diagnosis Active Patient Level Data entry. The created entry was consistent with the Release 2 of the HL7 QRDA 1 IG and the Release 3 of the IG is very different. This is what was created by the Bonnie tool… (third attachment)

Not only is the Diagnosis Active template ID missing the required extension ("2014-12-01" ) but, in the Release 3 of the HL7 IG, the Diagnosis Active section is now embedded in a Diagnosis Active Concern Act (2.16.840.1.113883.10.20.24.3.121). The beginning part of the entry should now look like this… (fourth attachment)

My view is that v1.4.1 of the Bonnie Tool provides only superficial support for Release 3 of the HL7 QRDA 1 IG.
