For NQF 0421 we encountered an issue while testing the QRDA-1 report for patient N GP Adult, ONE N on cypress.
In the cypress report we have an entry with value set code 2.16.840.1.113883.3.600.1.1525 and an associated reason (<entryrelationship/> tag) for Intervention, Order: Above Normal Follow Up. But there's one more entry with same value set i.e. Intervention, Order: Physical Activity Recommendation (code 304549008) where there's no reason (<entryrelationship/> tag) associated.

This is a bit of an issue for us to identify where to have <entryrelationship/> tag and where not when it comes to same value sets.

What is the correct intention?
