CMS 144 and 145 LVSD Moderate or Severe

      In Cypress C1 (Record Sample) we are asked to enter the nomenclature from the OID for the measure presented. CMS 144 and 145 require the entry of the diagnosis LVSD (moderate or severe). For these measures the specification offers 3 OIDs to identify this patient population.
      "Diagnosis: Moderate or Severe LVSD" using "Moderate or Severe LVSD Grouping Value Set (2.16.840.1.113883.3.526.3.1090) = [2 fully descriptive LVSD problems 1 Mod and 1 Severe]
      "Diagnosis: Left Ventricular Systolic Dysfunction" using "Left Ventricular Systolic Dysfunction Grouping Value Set (2.16.840.1.113883.3.526.3.1091)" = [1 generic LVSD problem thus requires the attribute below]
      Attribute: "Severity: Moderate or Severe" using "Moderate or Severe Grouping Value Set (2.16.840.1.113883.3.526.3.1092) = [generic qualifier 1 Mod and 1 Severe]

      Cypress is requiring us to use the 2 code/attribute option to meet this measure. (choose generic 2.16.840.1.113883.3.526.3.1091 and then add the generic quantifier to the dx 2.16.840.1.113883.3.526.3.1092). This is not something that we have programmed into our system because our Medical Problem diagnosing tool doesn't offer the ability to add additional non-problem attribute snomed codes to a problem. Instead it allow for very precise diagnosing by choosing the appropriate descriptive diagnosis found in 2.16.840.1.113883.3.526.3.1091. We would never advise clinicians to choose a generic code and then a qualifier (more work, more clicks, more errors).

      When we choose the descriptive code found in OID 2.16.840.1.113883.3.526.3.1091 the patient qualifies for the measure. Is there any way to still pass this measure without having to find a workaround to enter the two OID values. This rigid requirement isn't a very accurate test of an EHR's capabilities. We can program a change but it would never be used in real life, thus doesn't verify real use.
