CMS-157 QRDA Category III certification has an outcome that is inconsistent with other guidance on the measure. According to https://oncprojectracking.healthit.gov/support/browse/CQM-1675

"In version 4 of this measure which was published in May 2015, we have included the item count field which further identifies the unit of measure for the eCQM. For CMS 157, this would be looking at every instance of an office visit or face-to-face interaction encounter, and every radiation treatment management procedure."

This would indicate that the expected outcome (in my case IPP 3/DENOM 3/NUMER 1) is incorrect, only counting procedure_performed elements, and excluding encounter_performed elements. In my case this would result in IPP 10, DENOM 10, NUMBER 3.
