Steps to reproduce issue:
1. Connect browser to Cypress Validation Utility at https://cypressvalidator.healthit.gov/
2. Select test file CQM2016_20150101_to_20151231_GMH.xml file (attached, contains no PHI) to upload.
3. Select Annual Update Bundle = 2015 Bundle for the 2016 Reporting Period.
4. Select CMS Reporting Program = PQRS - Individual.
5. Select Document Type = QRDA Cat III (R1).
6. Check PHI Confirmation checkbox.
7. Press Submit button (settings above are captured in Settings.PNG attachment).

Expected result:
Test file is processed and any errors are shown.

Actual result:
Site returns a 413 Request Entity Too Large error (see Error413.PNG attachment).
