I am testing Manual Entry and I keep getting errors under the header "Missing or Duplicate Files" that say I have a template not complete. The most common of these is "diagnosis, active not complete". I have seen this in multiple measures.

This is a very vague error message and I have no idea what it means. I can clearly see that each of these "not complete" elements is documented and present in the XML I upload, so I have no idea why they are considered incomplete.
