While testing conformity for QRDA3 2026 for MIPS Individual and MIPS Group programs, it fails with the following error message - 'For CMS eligible clinicians and eligible professionals programs, all SEX codes present in the value set must be reported, even if the count is zero. If an eCQM is episode-based, the count will reflect the patient count rather than the episode count.'

Even though both SEX codes (male: 248153007 and female: 248152002) are present on the QRDA III file for all measure components (IPOP, DENOM etc.), this error message is displayed.

Secondly, when a QRDA III good known file from Cypress for any measure or a QRDA III sample file provided by CMS is uploaded on Cypress to test conformity, we get the same error.

Please help us understand the issue we are facing in this regard and how it can be resolved.

Following screenshots and QRDA III files are attached for reference:
      1. MIPS_GROUP - A generated QRDA III file from our system eCQM_GROUP_QRDA3_987654321.xml
      2. MIPS_INDIV - Good known file for CMS75 CMS75v14_689cc3688470c100810eb0d5.debug.xml
      3. HL7 Cat III Test - Sample file provided by CMS for 2026 2026MIPSAPPGroupSampleQRDA-III-v1.0.xml
