CMS 122 QRDA-1 patient name and data Issue

      Issue#1

      currently i select the certification type C1 and C2, then i download the patient zip file under download full test deck. this zip file contain 25 patients. as you show in attached image["QRD1.jpg"]
      and i also attached the expected result image. 
      when we import patient in our EHR we check the last name, first name and dob are exists in our DB then we merge the patient else we create the new one.

      Now the issue is that how we know the patient is duplicate ? where first name are change. 
      on which criteria we know the patient is duplicate ? [i also highlight the duplicate patient in QRDA1 image] 
      in this scenario the first name of some patient are change and last name & dob are same. 

      Issue#2

      in the download folder there are two files

      21_Vanessa_Simpson.xml

      22_Vanessa_Simpson.xml

      in the first file there is only diabetes problem, where as there is no information of encounter.

      when i import this QRDA-1 file in our EMR, so this problem goes under which encounter ? although there is no record of Encounter is mention in the File.

      For reference i also attached the both xml files.

       

      Patient Name->21_Vanessa_Simpson.xml  , Template Name->Z28 N Heart Adult

       

      so kindly guide me in this regard
