Validating a QRDA Cat1 R4 document against the 2018 CMS HQR_EHR Combined IG.

Found several patients that are not included in the IPP but files include Inpatient encounter (2.16.840.1.113883.3.666.5.307) and age is older than 18. Example attached cern_hcd_QRDA_394_1

Different issue in cern_hcd_QRDA_357_1 evaluated as Den Exc but a different outcome is expected:
Measure breakdown displays the following logic a TRUE, but the file does not have data supporting that fact:
Encounter, Performed: ICU Admission or Transfer (length of stay >= 1 day(s))" <= 1 day(s) starts after or concurrent with start of Occurrence A of $EncounterInpatient
