Medication Fulfilment History is missing in Cat 1 export in Cypress 3.2.1 for CMS156v5

      Fulfilment History is missing in exported QRDA Cat 1 document. Please find attached
      a) sample QRDA Cat 1 document for test patient "Catherine White"
      b) the patient's HTML file showing fulfilment history
      c) the patients list for the measure bucketing this patient for NUMER
      d) also, the debug Cat 1 file generated from "Known good results"

      I think this is a bug, please let me know if I am missing something.
