We are testing the files for our manual entry certification which is this Thursday. When we are putting in our codes required for the test for CMS139 Risk Fall Assessment, the first code required is for the Risk Assessment being performed and the second code required is a negation code stating the risk assessment was not performed. Our logic is such that you would not do an assessment and also state you did not do the assessment on the same day. Therefore, this is causing us to fail. Can you give guidance on this issue and how we should proceed. I am attaching screenshots showing the required codes in Cypress as well as the valuesets coding on the spreadsheet from our proctor.
