    • Interpretation of denominator exception for new measure

      QPP 509 denominator exception reads "Documentation of patient reasons for no examination i.e., refusal of examination OR lost to follow-up". We would like some additional guidance around what would constitute a patient lost to follow-up.

      Specifically, for "documentation of patients who relocated outside of the geographic area" - Should this include any address change? Or only address changes to a different zip code? To a different state?
