      Can a registered nurse, licensed practical nurse, clinical assistant (rooming staff) document the follow-up plan for a patient who had a positive depression screen on or up to two days following the eligible visit?  Or does the follow-up plan need to be documented specifically by the provider/eligible clinician who is billing for the eligible visit?
